Not Born in Australia
Any child, parent or carer not born in Australia must provide a copy of their passport or Australian Citizenship Certificate.
Not an Australian Citizen
Any child, parent or carer who is not an Australian citizen must also provide a copy of their visa.
Catholic Families
Where applicable, provide Baptism, Reconciliation, First Communion or Confirmation certificates, and Parish Reference.

Please complete the online Application for Enrolment Form linked below and provide the required documents to our office via email or in person. When completed, please return the form to our school via post, in person or via email to admin@ctk.wa.edu.au.
There is non-refundable processing fee of $35 that is required to accompany the enrolment form. This fee can be paid by cheque/cash/ or EFTPOS at our office, money order via post or electronically via internet banking. Please contact us for these details.
This form is an application only and does not mean automatic acceptance.
All enrolments are subject to an interview with the Principal.
